Justin Hernandez Obituary, Death; – The San Antonio Police Department has disclosed the identities of the three officers involved in the shooting death of a man during a mental health intervention late Saturday night.

In a news release issued on Tuesday, SAPD announced that the officers who discharged their firearms at a Knights Inn situated in the 4000 block of East Houston Street, close to Interstate 10, are Kevin Nguyen, William O’Brien, and Samuel Whiteside. Each officer has served with SAPD for two years, as stated by the department on Tuesday.

According to police reports, the officers were dispatched to the mental health call at the Knights Inn at approximately 11:56 p.m. on Saturday.

An initial report from SAPD indicated that the man was in one of the rooms, inflicting cuts on himself with a sharp object.

In an update provided near the shooting location, SAPD Chief William McManus initially mentioned that the three officers responded to a call made by another individual in the hotel room.

However, subsequent reports from SAPD do not reference any other persons involved in the incident.

Upon their arrival and after engaging with the man, police stated that he threatened the officers and charged at them while still wielding the cutting instrument.

In response, Nguyen, O’Brien, and Whiteside discharged their weapons, hitting the man, who was later pronounced dead at the scene, according to SAPD.

No additional injuries were reported.

The department has placed the officers on administrative duty pending further notice.

On Monday night, the Bexar County Medical Examiner’s Office identified the deceased man as Justin Hernandez, 34, ruling his death a homicide.

The SAPD’s Shooting Team, along with the department’s internal affairs team, will carry out separate investigations into the shooting.

The results of both investigations will be submitted to the Bexar County District Attorney’s Office, in accordance with SAPD protocol.
